Transcript Request

Faith Academy

An official transcript is a signed and sealed document which goes directly from the Faith Academy Guidance Office to a third party such as a school or employer. It must be mailed by us directly to that third party, OR if you wish to deliver it yourself, must remain in the sealed envelope in which you receive it.
